What You’ll Need to Do?
- Fill Out Forms in the Office: Upon arrival, you’ll be asked to complete a few forms related to your medical history, dental health, and insurance information. Our staff will be happy to assist you if you need any help while filling them out.
- Provide Insurance Information: If you plan to use insurance for your treatment, please be sure to bring the following with you:
- Insurance Card(s): To help us process your claims correctly.
- Birthdate and Social Security Number of the Insured Member: We will need this information to verify your insurance coverage and ensure we bill your insurance provider accurately.
